-Obtain temperature using thermometer of choice
3 -Position client in supine or sitting position
-Expose sternum and left chest
-Warm diaphragm by holding it in the palm of hand for 5-10 seconds
4 -Insert the earpieces of stethoscope into ears
-Place diaphragm over left chest wall
5 -If rhythm is regular, count the heartbeat for 30 seconds and multiply by two
-If the heartbeat is irregular, count for a full minute
6 -Replace client's clothing
-Assist client to a comfortable positions with wrist extended
-Place fingertips of your first two fingers along the groove at base of thumb on the client's wrist
7 If pulse is regular, count pulse for 30 seconds and multiply by two
-If pulse is irregular, count for one full minute. Initial pulse is counted as zero
8 -Without removing fingers from client's wrist, observe respirations
-Note depth, rhythm, and rate
-If rate is regular in an adult, count 30 seconds and multiply by two
9 -Assist client to a comfortable position
-Have forearm supported at heart level, palm up, upper arm completely exposed
10 -Locate brachial pulse to determine cuff placement
-Wrap deflated cuff snugly around upper arm with center of bladder over brachial artery
11 -Palpate radial pulse
-Inflate cuff until pulse disappears, note number
-Fully deflate the cuff and wait 1 to 2 minutes
12 -Repalpate brachial artery and place stethoscope diaphragm over site
-Inflate cuff to 30 mm of Hg above previous reading
-Slowly release valve so pressure drops 2-3 mmHg per second
13 -Note reading when first sound is heard (systolic)
-Note reading when last sound is heard (diastolic)
14 -Deflate cuff completely and remove from client's arm
-Assist client to comfortable position
-Discuss findings with client
15 -Take care of equipment and any waste appropriately
-Document findings
-Identify left arm or right arms used for BP
