4 Examinations Inspection, Inquiry, Palpation, Listening and Smelling
Inspection Refers to one of the 4 examinations that relates to vision. This is considered the 1st step in diagnosis. It is the most overlooked step in the clinic at PCOM, and probably the most used step in our every day life.
Tongue Diagnosis This is the most important part of inspection.
Inquiry This is the 10 questions involved in the "4 Examinations."
Palpation This is part of the 4 Examinations where we look at the 28 pathological pulses.
Listening and Smelling This is part of the 4 Examinations. This includes sounds of voice and smelling.
Spiritlessness During inspection, we see this pathology or a problem with this area primarily through the eyes. We can see this from eye movements, if they stare off into space, how a person moves, how they breath, if they are confused, and how they respond to the outside world.
Shen Disturbance During inspection, we see this pathology or a problem that would be related to schizophrenia.
False Spirit This usually happens when someone is dying and then all of a sudden their face will light up, they will be talkative and the cheeks will get red. They will seem to come to life and then they die. The Chinese name for this is jia shen.
Shape and Condition We are looking at constitution when we are looking at physical form for this assessment or the body appearance.
Strong Movement This movement is found in Yang conditions and excess conditions (shouting, irritable, ect.).
Weak Movement These movements are found in Yin conditions and deficiency.
Small Movements, Fidgeting Ask?
Internal Wind Tremors and convulsions would be seen in the body appearance would come from this pathology. These are usually from heat. The small tremors are from blood deficiency. Hemiplagia is also caused by this pathology. Name this kind of wind pathology (internal or external).
Atrophy SyndromeThe Chinese name for this is wei zheng. This also means "wilting." Weakness and lack of strength in the limbs, but not due to pain. In sever cases, there can be muscle atrophy. The muscles and sinews losing nutrition causes this. Often we will see this in deficiency conditions but it may be due to obstruction. This is known as____________ syndrome.
Five Phase Constitutional Types This theory relates to the 5 elements and constitution.
Facial Complexion During this part of inspection we look at the color, moisture, luster and normal complexion. This is known as ______complexion.
Pathological / Skin Blue-Green, Red, Yellow, White, Black are the 5 ________colors to the________.
Blue-Green Complexion This complexion will indicate cold, or some kind of stagnation from pain or stasis of blood (especially if it is more purplish-bluish). It could also be a cold causing stagnation.
Red This complexion is related to heat. Is it excess or deficient heat? The accelerated flow of blood give this color and brings it out to the surface.
Excess Heat If the whole face is red, this is due to:
Malour Flush Pink cheeks from deficiency heat.
False Shen The cheeks become red because of separation of yin and yang.
YellowThis complexion is associated with the spleen and with vacuity or dampness. Spleen qi deficiency will show a slight, pale________color in the face. We can also see it with dampness. Dampness literally accumulates which is thought to limit the spleen's transportation function. That would be more of a dim, luster_________color. Jaundice will also show this color from hepatitis.
Yin Jaundice This pathology known as______jaundice is about damp heat obstructing. If there is more dampness predominate, it is more_______jaundice. The color is a darker, smokey yellow. This can be seen from cerosis of the liver and pancreatic cancer.
Yang Jaundice This pathology will show an orange coloring of the skin that is a more damp heat condition.
White The element of metal (of the lung). We will see this in vacuity of qi and blood. This color is also known as "pale." It could also be from blood loss, deficiency or cold. Yang deficiency will also show this facial color. Blood deficiency will be a little dry and lusterless. Yang deficiency will be bright white (like snow...bright white and cold).
BlackThis color is associated with the kidney. We can have kidney deficiency that causes this facial color, or it could be kidney yin deficiency, as well. We would see swelling under the eyes with kidney yang deficiency. Blood stasis will show a dark, soot-like complexion which would be more purplish -_________ in late stage chronic diseases.
Spleen This facial area (see on-in in class notes). _________is in the middle of the face on the nasal area as listed in "Simple Questions."
Kidneys This facial area is on the chin as listed in "Simple Questions"
Lung This facial area is on the right of the face as listed in "Simple Questions."
Liver This facial area is on the left of the face as listed in "Simple Questions."
Heart This facial area is in the forehead area as listed in "Simple Questions."
Hair The condition of this will show the condition of the kidney essence and qi or liver blood. We look at the color, sheen, thickness and hair loss to determine the condition.
Eyes These are the orifice of the liver.
Spleen This part of the eye, the upper eyelid, relates to the____________.
Kidney The pupil of the eye relates to what organ?
Heart The inner and outer canthus relates to what organ?
Lung The sclera of the eye is related to what organ?
Liver The iris of the eye is related to what organ?

Teeth These are the extension of the bones and are governed by the kidney. If this area is normal, they are all present, bright white and clean.
Kidney Yin Deficiency Loose teeth or loss of teeth will be this pathology.
Stomach Fire Bleeding painful gums is related to this pathology.
Stomach This organ governs the gums.
Blood deficiency Pale gums or receding gums represent this pathology.
Spleen Deficiency or Stomach Fire This pathology shows bleeding gums, mouth ulcerations and is caused by heat (excess).
Throat The lung and the stomach channels go to this location. The lung opens to this location. It also produces the voice.
Excess Heat of Lung and Stomach Red, swollen, painful throat can be this pathology.
Lung and Kidney Yin Deficiency A chronic sore throat that is dry, not red or only slightly red could be this pathology.

In the upper limbs, it has more to due to the lungs and it will be more acute. Below the west (especially down into the feet), this is kidney edema. Spleen yang deficiency will affect the whole body. This is also more chronic).
