Two reasons why holes appear in defensive layers Active failure- unsafe acts committed by people who are in direct contact with situation or system. Not wearing a seat belt.
Latent Conditions- inevitable "resident pathogens" within a system
Laten Conditions Have two adverse effects. They can translate in to error provoking conditions within the workplace. ie; time pressure, under staffing,bad equipment
Secondly they can create long lasting holes in the defense ie: untrustworthy alarms,unworkable procedures.
CRM has three activities avoidance..entrapment..mitigating consequences
Error Avoidance Provides the best opportunity for trapping and preventing errors from becoming a catastrophe.
Entrapped Errors that are avoided are trapped at this level
Mitigation This is were errors slip through the first two phases. This is were emergency responders minimize effect.

Communication Freedom of question is encouraged..Inquiry is questioning a situation that causes concern. Advocacy is a statement of opinion that recommends a proper course of action. These two require the most attention and work.
