1. lower head of bed
2. raise opposite bed rail
3. roll client toward rail
4. place pad and bedpan under buttocks
5. raise head of bed
6. toilet paper, call light w/in reach (signal when finished)
7. give patient wet washcloth to clean hands
8. lower head of bed
9. roll client while holding bedpan
10. remove bedpan, pad (put in dirty linen)
11.empty pan in toilet, rinse, empty in toilet, and place in bottom drawer (pull drawer open with ungloved hand)
12. remove gloves & wash hands

DENTURE CARE

Supplies: Mouth swab, tooth brush/paste, gloves, paper towel, bibb, spit basin, water cup, dentures

1. Raise head of head 90 degrees
2. swab patient's mouth with mouth swab
3. give patient chance to rinse and spit
4. line sink w/ wash cloth or paper towels
5. run luke warm water
6. put tooth paste on tooth brush
7. Brush dentures
8. Rinse cup and fill with water
9. Place dentures in cup
10. clean up and store all supplies

COUNT AND RECORD RADIAL PULSE

1. place finger on thumb side of client's wrist
2. count beats for 1 full minute

1. have client cross their arms across chest

DRESSING CLIENT W/AFFECTED RIGHT ARM

Supplies: Shirt, pants, socks
1. give client choice of clothing
2. put on pants and socks first, pull up to thigh
3. remove gown from unaffected arm (left arm) 1st, then affected side (right arm)
4. dress affected side (right arm) 1st, then unaffected side (left arm)
5. when turning patient to dress, pull pants up
6. place soiled linens in hamper

FEEDING CLIENT

Supplies: Food, bibb, spoon, washcloth, napkin, water
1. set bed to 90 degree (upright position)
3. clean clients hands w/ washcloth
4. place bibb on patient
5. give water before feeding
6. give water every 3-4 spoonfuls, check for swallowing, talk with patient, encourage
7. wipe mouth & hands w/ bibb
8. record amount eaten (100%!!)

PARTIAL BED BATH

Supplies: 2 Basins, 2 washcloths, 4 towels, lotion, clean gown

1. remove gown on areas being washed, protect privacy with towel drape
2. client check water temperature
4. wash face, start with eyes, far eye first, use different part of cloth (NO SOAP)
5. dry face (pat dry)
6. expose arm and place towel under arm
7. soap wet cloth
8. wash, rise, dry arm, hand, and underarm (underarm last)
9. put on clean gown
10. ask patient to turn over and wash, rinse, dry back
11. apply warm lotion on back and pat off excess with towel
12. empty, rise, dry basin and store (ungloved hand open drawer)
13. dirty gown in hamper

MAKING OCCUPIED BED

Supplies: 1 fitted sheet, 1 top sheet, 1 pillowcase
*gloves*
1. make sure bed is flat
2. raise opposite bed rail
3. loosen top linen, have client roll to the bed rail, loosen bottom
4. tuck dirty linen under client, then place clean linen top and bottom
5. raise other bed rail, have client roll to that side
6. remove opposite rail, remove dirty linen, put on other side of clean linen (put dirty linen in hamper)
7. have client hold clean top sheet and remove dirty top sheet (put dirty sheet in hamper)
8. tuck in the foot of bed & fold corners
9. change pillowcase (turn pillowcase inside out and pull through)

BLOOD PRESSURE

Supplies: stethoscope, bp cuff, alcohol wipes
1. wipe stethoscope (both instructors and yours) ear and bell
2. follow pinky to find brachial artery, palpitate and inflate cuff until no pulse (160-180)
3. place diaphragm on artery and listen for first and last beats, S/D, give only EVEN numbers ~120/80
4. RECORD measurement
5. wipe stethoscope (both instructors and yours) ear and bell

MEASURE AND RECORD URINARY CATH OUTPUT

Supplies: Gloves, alcohol swabs, graduated cylinder

1. pour the contents from bag into graduated cylinder, DO NOT TOUCH SIDES
2. wipe tube with alcohol swab and fasten closed
3. make sure bag is fastened to nonmovable part of bed and not on floor
4. close toilet lid and place paper towel, sit container flat, read eye level
5. empty into toilet
6. rinse measuring container and pour rinse into tiolet, dry
7. store measuring container and supplies in bottom drawer, use ungloved hand to open drawer
8. RECORD in cc's (30 cc's = 1 oz)

WEIGHT

1. ensures client has their shoes on
2. walk to scale and set scale to zero
3. stand on side and assist
4. determine weight

2. raise side rail on side body will be turned
3. assist client slowly roll onto that side
4. adjust or place pillow under head
6. support back by folding pillow #1 (long) & tucking by their back and have them lay on it
7. adjust arm and shoulder so no laying on them
8. support top arm with pillow #2
8. support top knee flexed pillow #3 and ankle with pillow

PROVIDE CATHETER CARE FOR FEMALE

Supplies: basin, water, soap, 2 washclothes, 2 towels, 1 pad

1. have client check water temp
2. place pad under peri area
3. fold 1st washcloth, wet washcloth, then apply soap to each corner,
4. wipe each labia, hold cath and clean 3-4 inches down
5. fold 2nd washcloth, wet
6. wipe each labia, hold cath and clean 3-4 inches down
7. DRY AREA, COIL TUBE
8. remove dirty pad and washclothes
9. empty, rinses, and dry basin and put in bottom drawer, use ungloved hand to open drawer
10. dispose of dry linens in hamper

HAIR AND NAIL CARE ONE HAND

Supplies: comb/brush, water basin, 1 towel, 1 washcloth, 1 orangewood stick, 1 nail file, lotion
1. have client check water temp
2. soak hand in water
3. ask patient about their preferred hairstyle
4. use brush/comb to gently brush hair from ends to roots
5. clean under fingernails with stick wiping stick after each nail
6. dry fingernails and file as needed - dispose of stick & file
7. rub hand with lotion, wipe off excess with towel
8. empty, rinse, and dry basin and put in dirty area
9. dispose of dirty linens

PROVIDE FOOT CARE ON ONE FOOT

Supplies: 2 towels, 1 washcloth, soap, lotion, water basin, socks, shoes

1. have client check water temp
2. place foot in water and apply soap to wet washcloth
3. lift and wash foot, including between the toes
4. rinse foot in water, including between the toes
5. dries foot, including between the toes
6. apply lotion on top and bottom of foot (NOT between toes) and dry off excess
7. empty, rinse and dry basin put in dirty area
8. put sock and shoes back on
9. dispose of dirty linens

PROVIDE MOUTH CARE W/ TEETH

Supplies: emesis basin (c shape basin), tooth brush, tooth paste, small cup of water, bibb, wash cloth

1. raise head of bed 90 degrees
2. place clothing protector across clients chest
3. mositen toothbrush and apply toothpaste
4. clean mouth including tongue, surfaces of teeth
5. place toothbrush on paper towel
6. have client rinse, place basin under chin for spit
7. wipe mouth and remove clothing protector
8. empty, rinse, and dry basin and throw away toothbrush, place basin and supplies in bottom drawer (use ungloved hand to open drawer)
9. dispose of clothing protector

PROVIDE PERI-CARE FOR INCONTINENT FEMALE

Supplies: water basin, 2 pads, 4 washclothes, 2 towels, soap
*gloves*
1. have client check water temp
2. apply bath blanket/towel
3. place new pad under peri area
4. apply soap to wet 1st washcloth and wash genital area from front to back using clean area of cloth with each stroke
5. use 2nd washcloth to rinse (no soap) from front to back using clean area of cloth with each stroke and pat dry
6. raise side rail and have client turn over on their side
7. apply soap to wet 3rd washcloth and wash rectal area from front to back using clean area of cloth with each stroke
8. use 4th washcloth to rinse (no soap) from front to back using clean area of cloth with each stroke and pat dry
9. repositon client
10. empty, rinse, and dry basin and put in drawer (use ungloved hand to open drawer)
11. place 2nd clean pad under patient and remove dirty one
12. dispose of dirty linens

TRANSFER FROM BED TO WHEELCHAIR USING TRANSFER BELFT

Supplies: transfer belt, wheelchair
1. place wheelchair on side of bed and make sure wheels are locked
2. place footrests up or remove
3. make sure bed is locked and in lowest position
4. assist client to sit, apply shoes and belt
5. provide instruction to stand (1,2, 3, on 3)
6. turn client so that their legs are against wheelchair
7. lower client into wheelchair
8. remove transfer belt
9. put feet on footrest
