Controlling the Environment Never leave your keys laying around where the client can get them
Never leave restricted items unattended
Watch your supplies
Mirrors can shatter and cut the client
-Watch the conditions of the floor (slippery)
Electrical hazards
Observe food regulations (diabetic, allergic)
Structure tx for cognitive levels
Elopement Secure windows and doors
Be cautious when escorting the client
Notify the staff before taking the pt. off the ward
Never take the client on trips away from the facility alone
Suicide Be alert for suicidal behaviors
-Supplies missing that could be harmful
Remove potentially harmful objects
Exclude patient from outings
Assault The patient will not receive OT until medicated
The exceptions to this rule include
-no warning of the assault
-unprovoked
-if the patient has no memory of the assault
Steps to follow in case of AssaultCall for more staff immediately
Remove other patients from the area
-high level functioning patients can escort others
Attempt to calm patient
-calm soothing voice
-get patient to explain what is troublesome
-Do NOT try to overpower patient
Follow up on the assault by documenting the incident and getting others in the group to express their feelings
Awareness Be aware of:
Anxiety
Defensive Behaviors
Acting Out
Two forms of Aggressive Behavior Verbal and Physical
Anxious Level Notice a change in the persons behavior (pacing, wringing of the hands, more pronounced movements)
Respond by actively listening to the problem
Set Limits Be clear, concise, and reasonable. ALWAYS follow through on what you say
Therapeutic Use of Self Communicate with the person
Participate in deep breathing to reduce tension
Inform the client as to what you will be doing
Be careful of your nonverbal language
Keep a personal space of 1 1/2 to 3 ft
Non-Verbals Body Positioning and Posture
Speech tone
