Pulmonary Edema (adult) Lasix at patient's single (not daily) oral dose IV slow, to a max of 120mg For patients not on lasix, 40mg
Agitated Delirium (adult) Midazolam (Versed) 5mg IM or 2mg IV slow
Crush Syndrome (adult) entrapment >4 hours, consider Dextrose 50% to IV fluids
Torso Injury (adult) if refractory to the first 1000ml NS, consider additional fluid challenges
Allergic Reaction / Anaphylaxis (peds) epi (1:100,000) 0.1mg IV over 5-10 minutes. * epi (1:10) 0.1 mg (1 ml) in 100ml NS, drip 120 gtts/min (10ggt set)
PSVT (peds) for peds that do not convert with vagals, give Adenosine 0.1 mg/kg followed by a 10ml flush. if no conversion after 1-2 minutes give 0.2mg
Hypoperfusion dopamine drip
Organophosphate / Nerve Agent (peds) severe Pralidoxime (2-PAM) autoinjector in peds over 15kg
Croup / Epiglottitis (peds) Epi (1:1) 5mg undiluted via NEB
Asthma / Brochiolitis (peds) Mag Sulfate 25mg/kg in 100ml NS IV drip 30ggts/min (10ggt set)
Pain Management (peds) additional doses
Nausea / Vomiting (peds) <8kg contact OLMD
Crush Syndrome (peds) entrapment > 4 hours consider D50 to IV fluids
Torso Trauma (peds) for patients 10 years old or older, consider permissive hypotension
Adenosine for peds- if no conversion with vagals, 0.1mg/mg rapid with 10cc flush, if no change 0.2 mg/kg

Magnesium Sulfate (adult) Stable polymorphic Vtach with pulse- 2 grams in 100ml NS IV drip 60gtts/min (10gtt set)
Magnesium Sulfate (peds) Stable polymorphic Vtach with pulse- 50mg/kg in 100ml NS IV drip wide open (10gtt set) Asthma/Broncospasm- 25mg/kg in 100ml NS IV drip 30gtts/min (10gtt set)
Mark 1 Kit all peds
Nitroglycerin meds similar to viagra within the last 7 days
Ondansetron for peds <8kg
