A 26 year old college student complains that she has a cold. She has been sick for about 24 hours. She is not coughing and does not have a sore throat, but she feels slightly feverish with a distinct chilly sensation that causes her to shudder during the interview. She states that she has been sweating all day, even while she feels chilled. her tongue is a normal pink color, is not swollen, and has a thin glossy or moist coat. her pulse is floating and moderate.
Tai Yang Pattern with external deficiency.
You are called upon to treat a 69 year old woman who has been hospitalized for congestive heart failure. her lower extremities are swollen with edema. her complexion is pale and bright. She does not have difficulty breathing but she complains of fullness in her chest. Her limbs are so cold that she has to wrap herself up in her blankets like a mummy. Her lips are purple and her tongue is plae and blue with a moist(glossy) white coat. There are purple spots in the sides of her tongue. Her pulse is rapid and forceless, and periodically skips beats. You determine that she is suffering from blood stasis. But what organ pattern is she suffering from?

A 42 year old female nurse complains of migraine headaches that began 1 year ago with the birth of her youngest child and have progressively become worse. The migraines are severe and occur 2-3 times a week. She states that the headaches have weakened her considerably and have prevented her from functioning normally. Although the headaches focus on the right side, her entire head feels heavy. The headaches usually cause nausea, and occasionally vomit, even when her stomach is empty. Sometimes the vomit has a slimy appearance, like mucus. She states that she does not feel particularly hot or cold, and denies night or day sweat. She states that when she first lifts her head in the morning, she sometimes has a sensation that feels as if the room is spinning. On several occasions, this sensation has been so severe that she had to remain in bed for several hours before she could get up and maintain her balance.
Her mouth tends to be dry and she relieves this by sipping liquids. She states that she often has a dull tight sensation in the upper part of her stomach, and this weakens her appetite, even when she is not experiencing a headache. She denies lower back or knee pain, palpitations, and insomnia. She does not have a bitter taste in her mouth. her tongue is pale with a thick glossy (moist) coat. her pulse is wiry.

A 19 year old male has been brought to a hospital in china with acute appendicitis. he has severe abdominal pain and distention that is worse with pressure and a fever of 40 C (104 F). He has not been able to move his bowels. His pulse is deep, forceful and full and his tongue is red with a dry, dark yellow coat.

A 25 year old woman seeks acupuncture for menstrual irregularity and pain. She states with some exasperation that at this point she cannot tell when her period is due, so irregular has it become. When it does arrive there is severe pain for two to three days prior to bleeding, with gradually diminished pain on the first day. She states that she also feels tenderness in her breast that is so severe she has to loosen her brazier. She answers affirmatively when asked if she experiences discomfort in her "rib-sides". She seem irritable and exasperated, and says that with her periods coming and going unpredictably, she feels that she cannot live a normal life. Her physician has told her that she has situational depressoin, and has recommended prescribing prozac, but she wishes to try chinese medicine before taking a antidepressant. Her pulse is tight and her tongue is normally colored with a light moist white coat.
Binding Depression of Liver Qi
55 year old female sales representative seeks treatment for reduced sexual drive. She states that her health is overall quite well, but that recently her gynecologist examined her and states that there was atrophying of the vagina. She has no children. She is unmarried, but has recently begun a relationship with a sales representative from a rival firm. She is somewhat overweight and her face is pale She has gray circles under her eyes, denies sensations of cold or heat; no abdominal pain, but she complains of an increased girth of the lower abdomen, but without distention or discomfort. Her stools, urine, appetite, thirst show no abnormal signs. She stopped menstruating several years ago, and has never had hot flashes. She was successfully treated 20 years ago by chinese medicine for amenorrhea, and never had any problems subsequent to that time. She has soreness and chronic pain in her back, and both knees, which are worse after working or exercising. Her pulse is thin (fine) and forceless, 72 beats per minute, and her tongue is pale, moist glossy and swollen.
Kidney Yang Deficiency
a 45 year old factory worker has come to the emergency room of a large city and has been diagnosed with acute cholecystitis. You examine her as she waits to be admitted. She has a fever of 38 C(101F). She feels feverish, and she says that she has felt feverish over the last twelve hours. nevertheless there were several brief intervals where she states that she felt chills to the point of shaking. she has severe pain in the right upper abdominal quadrant, radiating up into her right rib cage and shoulder. She has a bitter taste in her mouth and her urine has been dark. She is constipated. She has a headache, and feels very restless and nauseous. her pulse is wiry. Her tongue is red with a clean, dry, white coat.
Shao Yang Disease
An 18 year old male student, a hockey player, is about to begin college. He has just been diagnosed with Epstein-Barr Disease. He has been prescribed amoxicillin. he feels feverish, but does not feel chills. His throat is very sore and swollen, but he is not coughing. He sweats profusely during the day and is thirsty with a preference for cold fluids. he is not coughing or wheezing, and states that he has not had any respiratory distress since the condition began. He denies fullness in the chest or rib side pain. He is not nauseous, but his appetite is considerably weakened. His stools are normal, but his urine is dark. His pulse is surging (flooding), and fast, his tongue is red with a dry white coat.

A 7 year old boy is brought to an acupuncturist for treatment of a chronic sore throat. The boy had a streptococcus infection 6 months now, which was successfully treated with antibiotics, but he has not felt well since that time. The sore throat is mild but persistent, and seems worse at night. He also experiences night sweats and sometimes runs a low fever in the late afternoon. His palms and soles feel warm to the touch, and his mother states that sometimes he has night terror -- waking up in the middle of the night terrified after a bad dream. He states that his mouth and throat feel dry. He had never had a problem with bedwetting and does not get up in the night to urinate. His pulse is empty and fast. Upon examination his throat is red and only slightly swollen, and his tongue is red with a very light white coat.

A 25 year old social worker had a very bad flu, which kept him in bed for several days with chills, fever, a runny nose, and a sore throat. Eventually, the fever subsided and the upper respiratory symptoms went away. But the patient stills feels tired and weak, and his stools are loose. he no longer feels the chills he felt earlier, but his extremities are quite cold. he experiences a dull pain in the abdomen that is relieved by pressure. While he had the flu, he was taking apirin.
