Terms in this set (9)
What are the possible immune related reactions to transfusion?
- wrong blood
- Febrile Non-Hemolytic Reaction
- Allergy or anaphylaxis
- Delayed Transfusion Reaction
What Ig is involved in immediate hemolytic reaction?
IgM resulting in complement activation
Occurs when wrong ABO blood is given
General signs of shock
What Ig is involved in delayed hemolytic reaction?
IgG resulting in extravascular destruction
Occurs due to secondary response to antigen from previous exposure e.g. RhD
Symptoms and signs may occur 1 week after
- renal failure
other signs of shock etc
Confirm with Directs Coomb's Test
What causes non-hemolytic Transfusion Reactions
Due to presence of leucocyte antibodies sensitized against donor's proteins, resulting in release of pyrogens
What are some signs of NHTR?
Chills and Rigors
Treat with aspirin
What causes of TRALI?
HLA antibody in donor reacts with HLA antigen on patient's WBC
- immune complications in lung leading to damage
- Aggregates of WBC stuck in pulmonary small capillaries causing lung damage
- Usually when there is multiparous woman donor given to men
What are some signs and symptoms or TRALI?
Perihilar and lower lung field shadowing on CXR
What are some non-immune related adverse effects of transfusions?
Decreased Calcium (anticoagulant depletes calcium)
increased potassium (leakage)
