Cardiac Medications

Alpha-Blockers

Alpha (fight or flight): othostatic hypotension, dizziness
Angiotensin II-Receptor Blockers
-SARTANS (Losartan)
Losartan
Angiotensin II Blocker: full effects may not be seen for 3-6 weeks, hyperkalemia, decreased renal function
Beta-Adrenergic Blockers
-LOLS (Metoprolol)
Metoprolol
Beta-adrenergic blocker (fight or flight): bradycardia, hypotension, wheezing, GI complaints, fatigue, depression
Calcium Channel blockers
-PINES (CARDIZEM)
Cardizem
Calcium channel blocker: contraindicated in acute MI, hypotension, heart blocks; hypotension and bradycardia
Uses: HTN, HF, Arrhythmias, PAD
Centrally acting agent
Clonidine: IV, PO, ONLY transdermal patch for HTN; rebound hypertension, drowsiness
Diuretics
Loop diuretics (Bumex, Lasix): Give IV slowly, check potassium levels, I/O, daily weights

Thiazie (HCTZ, Diuril): HF/HTN
Coumadin Considerations
INR: 2.0-3.0 for DVT
2.5-3.5 for mechanical valve
Antidote: INR >3.5 -> Vitamin K
Heparin Considerations
PTT has to be elevated (25-39 secs normal)
Antidote: PTT>100 -> Protamine Sulfate
