even distribution of eyebrows and eyelashes, no ptosis, conjunctiva clear, sclera white, cornea smooth, iris (color) bilaterally, cornea light reflex symmetrical, pupils equal, round and retract to light, pupils move parallel in 6 cardinal fields of gaze, fixed steady gaze, red reflex present bilaterally
equal size shape and height bilaterally, alignment with eye, external ear intact
tympanic membrane is pearly grey, concave, translucent cone of light bright bilaterally, visible Umbo, handle of malleus, short process of malleus, membrane moves with insufflation
symmetrical, no deformities, no lesions, mucosa & terbinates pink, moist, no exudates, no septal deviation, no perforations
Lips: Symmetry, Color, Swelling, Lesions, Movement of Lips with Speech, Smile, Cry
Buccal Mucosa: Pink, Moist, Presence of Lesions
Gingiva Pink, Hugging Margins of Teeth, No Bleeding
Salivary Glands: Sublingual and Parotid
Teeth: No carries
Oropharynx: Tonsilar Pillars Arched, Pink, Moist, Tonsils Sized 0-4, Equal, without Exudate, Uvula Midline, Pink, Elevates with Gag (CNX), Epiglottis Not Usually Visible
Tongue: Size, Color, Presence of Lesions, Surface Texture, Movement or Atrophy (CN XII)
Palpate Hard and Soft Palate
Identify Loose Teeth, Dental Appliances
Palpate Areas of Swelling, Abnormality
occipital, post auricular preauricular, tonsillar, submandibular, submental, superficial cervical, posterior cervical, deep cervical, supraclavical nodes non palapable bilaterally
Trachea Should Be: Midline, Non Tender, Firm Without Areas of Swelling, Hyoid, Thyroid and Cricoid Cartilages Should Move with Swallowing
Do with Tuning fork 512Hz
Conductive: Bone conduction > air conduction
Sensorineural: Air conduction > bone conduction
