endometriosis, prostate cancer
end in opin - Somatropin, Protropin, Norditropen
monitor with GH?
glucose (hyper) and thyroid function
Mecasermin (Increlex) used? administered?
used in kids with short stature and severe deficiency of IGF, administered once daily
monitor with Mecasermin?
hypoglycemia, headache, dizziness, vomiting, tonsil swelling
ADH (Vasopression) maintains?
fluid homeostasis, holding onto fluid
Deficiency of ADH?
diabetes insidious, treat with DDAVP or synthetic vasopressin, monitor for hypertension
how is DDAVP given? watch for?
oral, IV, intranasal - watch for water intoxication(fluid overload), drowsiness, headace, lislessness
too much ADH means?
SIADH - would see high BP, fast HR, edema (fluid overload)
calcium homeostasis - thyroid out of whack so does calcium
iodine required for production, major hormone secreted by thryoid, converted to T3 before it enters the cell
iodine required for production; more biologically active than T4
T3 and T4 RT to protein?
highly protein bound (Inactive when stuck to protein), issue with protein/albumin will cause T4/T3 excess and cause hyperthroidism
thyroid controlled by?
negative feedback loop
high iodine levels?
temporary decrease in thyroid activity - turns itself off
exposure to cold RT thyroid?
stimulation for increase thyroxine production = increased cellular functioning = increased heat production
autoimmune - body sees T3/T4 as a problem and attacks them
sx of hypothyroidism? labs? treat?
weight gain, constipation, sluggish, fatigue - increase of TSH - synthetic T3 or T4
levothyroxine comes as? (synthetic T4)
Levoxyl or Synthroid - DO NOT SWITCH BRANDS
nurse tips for Levothroxine?
start low, go slow, check TSH every 6 weeks, lifelong, take on empty stomach, allow 3 hours between meds and antacids
side effects for Levothroxine?
weight loss, headache, tremors, nervousness, heat intolerence, insomnia, menses changes, HTN - ask about sleep patterns, change in menses, weight changes
Levothroxine and Vit K?
breaks down vitamin K so increases the effects of Coumadin so monitor bleeding, bruises, low BP, hypotension
no source for metabolism in body, need to be treated with IV T3/T4 - typically from high iodine exposure
hyperthyroidism caused by? most common?
hypersecretion of thyroid hormones, thyroid or pitutiary tumors or pregnancy - Graves Disease
rare, life threatening extreme case - fever, cardiac and CNS effects, pt is as amped up as you can get
symptoms of Hyperthyroidism? TSH looks?
sweating, tachycardia, tremors, palpitations, bug eyes - usually under 0.5
immediate tx of hyperthyroidism?
iodine - simmers pt down(stop production of T3/T4), synthetic T3/T4
pharmacological tx of hyperthryoidism?
Propylthiouracil (PTU), Methimazole - inhibit iodine atoms into T3 and T4
PTU and methimazole nursing tips?
monitor for fever >38, jaundice, dark urine, light stool, elevated LFT, do not use in preggos, monitor bleeding, may give with beta blocker to decrease tremors and agitation
used to treat quickly when thryoid crisis, pt needs to void regularly, limit contact, increase fluids so radioactive particles is not near genitals, everything in biohazard bag
Lugol's iodine solution
reduces thyroid gland prior to surgery, used in emergent tx of thyrotoxicosis - monitor for mouth ulcers/gastric irritation, avoid food high is iodide, increase fluids
adrenal medulla produces?
epinephrine (SNS stimulant) Norepinephrine
increase blood sugar - causes bone breakdown (monitor bone density), monitor for edema, monitor/report temp
95% is aldosterone (makes you hold salt - where salt goes water goes)
mostly androgens(males) - help initiate puberty
replacement for those with adrenocortical insufficiencfy, used to reduce inflammtion in asthma, pre bone marrow transplant
primary vs secondary adrenocortical insufficiency?
addisons - does not naturally produce cortisol , sec - abrupt withdrawal of steroids
side effects of corticosteroids? KNOW
mood swings, hyperglucemia, PUD, hypocalcemia, hypokalemia, fluid retention, increased risk for infection, hirsutism, weight gain, bruising, OP, cataracts, muscle wasting
nurse tip for corticosteroids?
give lowest dose, every other day dosing, taper dose after 7-10 days, give with food, don't give immunizations, monitor wound healing
used as replacement in adreocortical insufficiency, enhances reabsorption of Na+ and H2O and excrete K+ - no anti inflam effects, monitor sodium/potassium labs
anti adrenal agents?
given in cushings disease - overactive ACTH from pituitary, adrenal tumors, long term corticosteroid use
sx of cushings
osteoporosis, HTN, infection/poor wound healing, obesity, moon face/buffalo hump
cushing med - blocks the synthesis of cortocosteroids - cant use in preggo
used to treat Cushing caused by adrenal tumors/cancer - side effects: anorexia, nausea, vomitting, depression, lethargy, dizziness
anti-adrenal used for diagnostic purposes - measure ACTH 8 hours later to determine if adrenal glands responded to inhibition, used off label to tx Cushings
