History: 33 year old female reports pain in her neck, shoulders, upper back, lower back, knees, calves present for 6 months. She says "I feel like I've been run over by a truck." She has trouble falling and staying asleep and awakens from sleep unrefreshed. She is stiff all over in the mornings and fells like her knees are sometimes swollen. She feels tired and reports trouble concentrating. Her symptoms are worsened by weather changes and activity. She does not exercise. She has missed a lot of work and thinks she might lose her job.

Other symptoms: Hands and feet feel cool but don't change colors, she has occasional headaches. Otherwise, her review of systems is negative.

Past Medical History:
Migraine headaches
Irritable bowel syndrome

Family History:
Her mother has migraine headaches and hypertension.
Her father has chronic low back pain.

Physical examination:
5'4", 134 pounds. Blood pressure, pulse normal
Skin: Hands and feet are cool to touch but with normal capillary refill. No rashes.
Neurologic: Her deep tendon reflexes and muscle strength are normal. Her sensation to light touch and proprioception are normal.
Musculoskeletal: She has 15 tender points on palpation. Examination of joints is otherwise normal

All other systems normal on exam.

She returns 3 months later. She has seen a counselor and has changed some of her sleep habits. She is able to fall asleep better and to manage days better when she has a lot of pain. She is walking for 30 minutes about 4 days a week. Overall, her pain decreased about 30%. She's missing less work now. However, she would like more relief and is now open to starting a medication.
