Class 2 Mechanism
Decrease SA and AV nodal activity by decreasing cAMP and decreasing Ca2+ currents. Suppress abnormal pacemakers by decreasing slope of Phase 4
AV node is particularly sensitive - increased PR interval. Esmolol very short acting
Class 2 Use
SVT, ventricular rate control for atrial fibrillation and atrial flutter
Class 2 Toxicity
Impotence, exacerbation of COPD and asthma
Cardiovascular effects: bradycardia, AV block, HF
CNS effects: sedation, sleep alterations
May mask signs of hypoglycemia

Adenosine Mechanism
Increases K+ out of cells which hyper polarizes the cell and decreases calcium current
Adenosine Use
Drug of choice in diagnosing / abolishing SVT. Very short acting ~ 15 seconds
Adenosine Toxicity
Effects blunted by theophylline and caffeine (both are adenosine receptor antagonists).
Flushing, hypotension, chest pain, sense of impending doom, bronchospasm
Mg++ Use
effective in torsades de points and digoxin toxicity
