For STEMI & NSTEMI
1st tx option; when to do a CABG? (6)
PCI w/ stenting
- L main dx
- 3 vessel dx
- 2 vessel dx in DM
- >70% occlusion
- pain despite txt
- post-MI angina
NSTEMI discharge Rx?
*B*eta-blocker (Metoprolol, Labetalol)
*A*CEI if CHF or LV dysfxn
*H*eparin (while in hospital)
CP w/out ST elevation & normal cardiac enzyme (how many repeats)?
normal cardiac enzyme x 3
if both negative, then unstable angina
Unstable angina workup (4)? what if the result is positive?
1. Exercise EKG
2. if EKG not possible, exercise echo
3. if pt cannot exercise, do chemical stress test (Adenosine/Dobutamine; Thallium-Dipyridamole)
4. multiple gated acquisition scan MUGA is nuclear medicine test that shows perfusion of the heart
