Obtain the medication from the storage area. Read the label carefully, comparing it with the order. Follow the seven rights.
Calculate the dosage, if necessary, and wash hands.
Prepare the medication:
a. take the bottle cap of and place it inside-up on the counter.
b. If in pill or capsule form, pour the desired amount into the cap. Then pour the medication into a cup.
Place the medication container on a tray and ordered dose in a medicine cup. Place a cup of water on the tray for the patient to drink with the medicine, if allowed. Read the label a second time and compare it with the order.
Take the medication tray to the patient and confirm the patient's identity. Read the label a third time and compare it to the order prior to administering.
Explain to a patient the rationale for performance of the procedure, showing awareness of the patients concerns related to the procedure being performed
Incorporate critical thinking skills when performing patient care. Give the patient the medication and offer the cup pf water, if allowed.
Discard any disposables and return the medication container and tray to the proper storage area. Read the label for the fourth time.
Record the information in the medication record. Sign off the order on the medication order sheet or chart.
