Indications: Routine antepartum prevention at 28 weeks of gestation in women with Rh- blood; suppress antibody formation after birth, miscarriage, pregnancy termination , abd trauma, ectopic pregnancy termination, abd trauma, ectopic pregnancy, amniocentesis, version, or chorionic villus sampling.
Contraindications: Myalgia, lethargy, localized tenderness and stiffness at injection site, mild and transient fever, malaise, headache; rarely nausea, vomiting, hypotension,tachycardia, possible allergic response.
Nursing Implications/teaching: Give standard dose @ 28 wks as prophylaxis or after an incident of exposure risk that occurs after 28 weeks (amniocentesis, 2nd trimester miscarriage or abortion, and after version). The medication is made from human plasma (jehovah witness consideration) The risk of transmitting infectious agents (viruses- which cant be eliminated completely).
Verify that the woman is Rh - and hasn't been sensitized if postpartum Coombs' test is negative and the baby is Rh +.
Indications: treats & prevents seizures. CNS depressant; relaxes smooth muscles, including the uterus.
Contraindications: Maternal- hot flashes, sweating, burning at IV site, N/V, dry mouth, drowsiness, blurred vision, diplopia, headache,ileus, generalized muscle weakness, lethargy dizziness, hypocalcemia, dyspnea, transient hyptotension, Some reactions may subside when loading dose is completed.
Intolerable- Resp rate is <12 breaths/min, pulmonary edema, absent DTRs, chest pain, Severe hypotension, altered LOC, Extreme muscle weakness, urine output less than 25-30 ml/hr or less than 100 ml/4 hrs
Serum Mg level of 10 or greater
Fetal (uncommon)- Decreased breathing mvmt, reduced FHR variability, Nonreactive NST.
