Patient physically completes all of the activity without assistance, within a normal time frame, uses no equipment, and is safe.
-No verbal cuing, demonstration, or prompting needed
FIM 6/Modified Independence
Patient physically completes all of the activity without assistance, but needs extra time, uses adaptive equipment, or has mild safety issues.
-Patient can go home alone, but may need to be checked on occasionally by family members
-Last independent living level
FIM 5/ Supervision
-Patient needs verbal cuing or prompting for the activity but needs no physical assistance
-Patient needs a helper to be in the room/house with them during the activity but not necessarily standing close by
Patient needs help to get supplies, open containers, cut meat, or other general preparations
FIM 5/ Stand-by Assistance
Patient requires someone to be present during the activity and the helper must be within an arms reach of the patient for safety
FIM 4/ Contact Guard Assistance
Patient requires touch assistance from a helper only for balancing. This is a hand on the back or arm for steady purposes only. No lifting occurs here
FIM 4/Minimal Assistance-For ADLs
Patient completes 75-99% of activity without assistance. Needs assistance for 1-25% of activity.
FIM 3/Moderate Assistance-For ADLs
Patient completes 50-74% of activity without assistance. Needs assistance for 26-50% of activity.
FIM 2/ Maximal Assistance- For ADLs
Patient completes 25-49% of activity without assistance. Needs assistance for 51-75% of activity.
FIM 1/ Dependent/ Total Assistance
Patient completes less than 25% of activity without assistance. Needs assistance for 76-100% of activity.
FIM 4/ Minimal assistance-For Transfers
-Patient requires lifting of one limb only during transfer. No lifting assistance is given.
-Patient requires touch assistance from a helper only for balancing. This is a hand on the back or arm for steady purposes only. No lifting occurs here. (Contact guard assistance)
FIM 3/Moderate Assistance- For transfers
-Patient performs more than half the lifting effort during a transfer
-Patient requires lifting two limbs only during transfers. No lifting assistance is given.
FIM 2/Maximal Assistance- For Transfers
patient performs less than half the lifting effort during a transfer
FIM 1/Total Assistance-For Transfers
Patient requires a mechanical lift or two helpers to transfer.
FIM 0 (Zero)
Only given if on the initial evaluation the patient did not perform the activity. At discharge score would be changed to a 1 if activity did not occur.
