STATE LISCENCE NUMBER
The state number that a physician must obtain in order to practice.
EMPLOYER IDENTIFICATION NUMBER (EIN)
known as Fedeal tax identificaion number. Issued by the Internal Revenue Service
SOCIAL SECURITY NUMBER
typically not used on the claim form unless the provider does not have an EIN
PROVIDER IDENTIFICATION NUMBER(PIN)
number assigned by the insurance company to a physician who renders services to patients.
UNIQUE PROVIDER IDENTIFICATION NUMBER(UPIN)
number assigned to the physician by Medicare program.
PERFORMING PROVIDER IDENTIFICATION NUMBER(PPIN)
a physician has a seperate number for each group office /clinic in which he pracitces.
GROUP PROVIDER NUMBER
this number is used instead of the individual physician's number for the performing provider who is a member of a group practice that submits claims to insurance companies under the group name.
