Head and Face Assessmemnt
1. Inspects facial skin - note abnormal lesions
2. Inspect head for size, symmetry, midline position
3. Palpate lymph nodes-occiptial, post aruicular, posterior cervical, anterior cervical, pre auricular, tonsillar, submandibular, segmental and supraclavicular
4. Cranial Nerve 5-Trigeminal-palpate masseter muscle as patient clenches jaw-sensory- have patient close eyes, softly touch forhead, cheeks, nose and chin-
5. Cranial Nerve 7-Facial Nerve-assess symmetry as patient smiles, frown, lifts eyebrows, puffs out cheeks and puckers lips
Ear Assessment
*Inspect outer ear for skin lesions and symmetry
*Inspect auditory canal and Tympanic membrane (using otoscope)
*Palpate Pinne and Tragus for nodules and tenderness
*Cranial Nerve 8-Whisper Test (whisper a word and have patient repeat, one ear at a time)
Eye Assessment
*Inspects conjunctiva and sclera
*Cranial Nerve 2-Ocular-Testing visual accuity and peripheral vision, also checking pupillary response
*Cranial Nerves 3, 4, & 6 (Oculomotor, Trochlear and Abducens)-Testing for congugate gaze with Extra Ocular Movements (EOM)
Nose Assessment
*Inspection of nose-midline and straight
*Inspect nasal turbinates and septum (using light source)
*Palpate maxillary and frontal sinus
Throat and Mouth Assessment
*Inspect lips, teeth, gums, buccal mucosa, palate, tongue, floor of the mouth (under the tongue), posterior pharynx and tonsils (grade tonsils)
*Cranial Nerves 9 & 10- Glossopharyngeal & Vagus-Have patient say "AHHH"-soft palate and uvula rise symmetrically and Check Gag reflex (not need to be done, but needs to be mentioned)
*Cranial Nerve 12-Hypoglossal-Have patient stick out tongue and move it left and right
