Use document when 2 physicians examine you and state you are either terminal where death is "imminent" or in a "persistent vegetative state"

- "life-sustaining" procedures to be withheld or withdrawn
- patient indicated whether he or she wants feeding tubes withheld/withdrawn or continued if terminal or in "persistent vegetative state"

Document does not work when patient is:
- neither terminal nor in "persistent vegetative state"
- terminal but death not "imminent"
- pregnant
- if patient also has Power of Attorney for Health Care
- implementing wishes that will cause pain that can't be reduced by use of pain medication

- electroshock therapy
- experimental mental health, drugs, and treatment
- Admission to mental facilities

Health care professional is not liable as long as they follow the document or fail to follow through on revocation if they didn't know about the revocation. Health care professional is liable if they fail to follow document and make a good faith attempt to transfer patient to another health care professional who will implement the document
