Specific information regarding patient wounds by type (gun shot or shrapnel) report serious bleeding, along with patient blood type if known.

A= US military
B= US civilian
C= Non- US military
D= Non- US civilian

Include details of terrain features in and around proposed landing site. If possible, describe the relationship of site to a prominent terrain feature( Lake, Mountain, Tower).
Always call break after line 5 complete. Reason being the first 5 lines are the lines needed to get the bird off the ground. Also gives receiving end of the call a brief second to look over any information in first 5 lines that may have been unclear.
Be clear and accurate when reporting so the request is sent without confusion and ASAP. Time is of essence and needs to be sent ASAP.
