What are the causes of infectious mononucleosis?

What is it characterised by?
What are the common complications of IM?
severe pharyngeal oedema
antibiotic induced rash (80-90% with ampicillin)
prolonged postviral fatigue (10%)
What are the investigations?
atypical lymphocytes - common but not specific
heterophile antibody during acute illness + convalescence - monospot test - doesnt mean much if -ve
specific EBV serology confirms diagnosis
What is the management of IM?
Symptomatic - Antivirals not active against EBV
Penicillin if B-haemolytic strep from throat culture (ampicillin + amoxicillin cause itchy macular rash)
Glucocorticoids (short course prednisolone) if pharyngeal oedema is severe
