CPA is a process that occurs secondary to failed cardiac contractility, which results in one of which 4 dysrhythmias?
1. Ventricular asystole
2. Pulseless electrical activity
3. Pulseless ventricular tacycardia
4. V-fib
What is the aim of CPR?
Provide blood flow to heart and brain until restoration of spontaneous circulation
What is the initial CPR success rate in dogs? In cats?
ROSC occurs in about
- 32% of dogs
- 43% of cats
After CPR, what is the survival-to-discharge rate in dogs? In cats?
-5% of dogs survive to d/c
-8% of cats survive to d/c
What does the BLS (Basic Life Support) part of CPR consist of?
-Recognize CPA
-Start chest compressions
-Airway mgt
-Assisted ventilation
Is CAB or ABC better for vet med?
There is no evidence in dogs/ cats that one is better or worse
- If unwitnessed CPA or d/t pulm d'se >> ABC
- If d/t primary cardiac dse >> CAB
What is the ideal chest compression rate in dog/ cat?
At least 100 bpm
(Studies show 120-150 bpm have good CO and improved coronary BF)
What is myocardial blood flow determined by? How is it calculated?
-Myocardial BF is determined by the coronary perfusion press
-CoPP = Aortic diastolic press - Right atria diastolic press
What is cerebral blood flow correlated with? How is it calculated?
-CBF correlates with cerebral perfusion press
-CePP = MAP - ICP
How does the relationship b/w CBF and CePP change during CPA?
-Normal autoregulation >> Constant CBF when CePP ranges b/w 50-150 mmHg
- CPA disrupts autoreg >> Relationship b/w CBF and CePP becomes linear
By how much should you compress the chest in order to maximize vital organ blood flow during CPR?
Compress the chest by 1/3 or 1/2 of its width
(depth of chest compression is positively correlated with improved aortic press)
Why is CoPP based on diastolic pressures?
CoPP is based on diastolic press bc left ventricular myocardium gets perfused during diastole
(Vessels traverse epi-/ myocardium >> myocardial contraction during systole compresses arterial branches and prevents perfusion)
Where do the coronary arteries originate? Where do they travel from there?
The R and L coronary aa originate from the coronary sinuses at the aortic root >> Divide and extend along the epicardium >> Branch through myocardium
Explain the thoracic pump theory
Inc intrathoracic press >> Blood moves out of thoracic cavity
In what size of animals is the thoracic pump theory thought to occur? What about the cardiac pump theory?
Thoracic: BW over 15-20 kg
Cardiac: Smaller
