Physical Status Modifier
added to each reported anesthesia code to indicate the patient's condition at the time anesthesia was administered.
normal, healthy patient.
patient with mild systemic disease; e.g. anemia, chronic asthma, chornic bornchitis, diabetes mellitus, essential hypertension, heart disease that only slightly lmiits physical activity, obesity.
patient with severe systemic disease; e.g. angina pectoris, chronic pulmonary disease that limits activity, history of prior myocardial infarction, heart disease that limits activity, poorly controlled essential hypertensioin, morbid obesity, diabetes mellitus, type I with vascular complications.
patient with severe systemic disease that is a constant threat to life; e.g. advanced pulmonary/renal/hepatic dysfuntion, congestive heart failure, persistent angina pectoris, unstable/rest angina.
moribund patient who is not expected to survive without the operation.
declared brain-dead patient whose organs are being removed for donor purposed.
HCPCS Level II Anesthesia Modifiers

anesthesia services performed personally by anesthesiologist.
medically supervised by physician for more than four concurrent procedures.
monitored anesthesia care for deep complex, complicated, or markedly invasive surgical procedure.
monitored anesthesia care for patient who has history of severe cardiopulmonary condition.
medical direciton of two, three, or four concurrent procedures involving qualified individuals.
monitored anesthesia care provided by an anesthesiologist.
CRNA service with medical direction by physician.
medical direction of one CRNA by an anesthesiologist
CRNA service without medical direction by physican.
