Podiatric Medicine I Hyperkeratotic Lesions 1
Terms in this set (37)
What are the layers of the skin?
In which layer do most corns and calluses form?
How do corns develop?
localized forces to a bony area
How do calluses develop?
Broader forces over a large area
What are the three types of corns?
What are the four types of calluses?
-intractable plantar keratosis
-medial punch callus
What are the four main types of hyperkeratosis?
What charcterizes warts?
-may not be in weight bearing areas
-bleed upon debridement
-pain with lateral compression
What are knuckle pads?
-benign, smooth lumps located over joint
-secondary to repetitive microtrauma
In which layer of skin do knuckle pads develop?
Stratum basale and stratum spinosum
What is a heloma durum?
hard corn that forms as a result of concentrated pressure found on the dorsal, distal, or lateral/medial aspects of toes
What can happen to a heloma durum in a neuothatic patient?
heloma durum can ulcerate if not taken care of
How do heloma durums form?
pressure+friction from a bony prominence and a tight shoe
What types of pads can be used to avoid pain from a heloma durum?
-crest buttress pad
-tubular foam sleeve
Describe an aperture pad.
-for dorsal HD
-CAN be used on 5TH TOE
Describe a budin pad.
-for dorsal and distal HD
-reorients flexible hammertoe to lie normally (reduces friction)
-NOT EFFECTIVE for 5TH TOE
Describe a crest pad.
-for distal HD
Describe a crest buttress pad.
-dorsal and distal HD
-NOT EFFECTIVE for 5th TOE
Describe a silopost sleeve.
-Slip for toe that contains a silicone pad.
-for dorsal and lateral HD
-CAN use on 5TH TOE
Describe a tubular Foam Sleeve
similar to silopost sleeve, but less comfortable.
What are three types of karatolytic therapies?
How does salicylic acid remove heloma durum?
-softens and sheds the stratum corner that builds up to produce the corn
-be careful of burns
How does ammonium lactate help with heloma durum?
-Increases skin moisture
-decreases epidermal keratinization
How does urea help with heloma durum?
Denatures proteins in heloma durum and moisturizes it.
What is a heloma molle?
Soft corn that commonly occurs in fourth interdigital space. Really just a hard corn that has absorbed moisture over time
Why are heloma mollies dangerous?
They can ulcerate and spread infection. Can be very painful
What are the main steps to managing a heloma molle?
3) educate patient
What types of pads are commonly used for heloma molle?
What is an interdigital pad?
Pad to put in-between toes to avoid interdigital friction from bony prominences.
What is the point of lambs wool?
relieve pressure and friction in heloma molle
1) separate digits
2) wick away moisture
3) harder to displace
What is a listers corn?
Heloma durum on the 5th digit
What is intractable plantar keratosis?
A deep seated callus usually found under the distal metatarsal head.
Why are intractable plantar kurtoses hard to treat surgically?
Usually the underlying bony problem is complex.
What 5 pathophysiological things contribute to an IPK?
-prominent metatarsal head
-retrograde force from proximal phalanx
-atrophic plantar fat pad
What types of pads can be used for an IPK?
-budin pad (reduce digital pressure)
-crest-buttress pad (reduce digital pressure)
-insoles (w/ or w/o cutouts)
What is a medial pinch callus?
Callus located on the medial aspect of the hallux or 1st metatarsal head.
What causes heel calluses?
shoes with weak heel support and protection
