St. John's Wort
Adverse reactions: Dry mouth, constipation, GI symptoms, skin rash when exposed to sunlight.
Teaching: Decrease the effectiveness of oral contraceptives, warfarin, digoxin, calcium channel blockers, steroids, HIV drugs and cancer drugs.
Adverse reaction: Nervousness, agitation, insomnia, diarrhea, HA, Heart palpitations.
Teaching: Do not take with MAO inhibitors. Can alter affects with heart medications. Can increase the risk of bleeding when taken with blood clotting drugs.
Adverse Reactions: GI symptoms
Teaching: Increase risk for bleeding. Can cause hypoglycemic effects.
Adverse reactions: Bitter taste, Mild GI symptoms, fever.
Teaching: Chronic use of this can decrease positive effects of medication for TB, cancer and HIV.
Adverse reactions: Mild GI effects, can decrease PSA marker to detect cancer.
Teaching: Possible additive effects with Finasteride.
Adverse reactions: Dry flaky skin, jaundice, liver damage.
Teaching: Can cause sedation when taken concurrently with CNS depressants.
Adverse reactions: GI distress, lightheadedness, HA, rash, wt gain.
Teaching: Increase the effect of antihypertensive medications. Increases hypoglycemia in client taking insulin or oral medications.

Teaching: May cause increased risk of bleeding in clients taking NSAID's, heparin, and warfarin.

Treats: Increase BP, fluid retention, Increased level of hormone aldosterone.
Teaching: Avoid substitutes that contain potassium.
Instruct clients to maintain adequate fluid intake by consuming at least 2000 to 3000 mL of fluid daily.
Treats: hypertension, fluid retention.
Teaching: Take medication in morning and second dose no later than 2pm, Consume foods high in potassium, Maintain adequate fluid intake 1500mL, If GI upset occurs, take with or after meals.
