Details of a DEA form 222
- includes 2 carbon copies (the original, plus 2 attached 2 copies)
- the first page (the original) is brown
- the second page (the first carbon copy) is GREEN
- must be forwarded to the DEA by the drug supplier
- the third page (the second carbon copy) is BLUE
- must be retained by the PHARMACY
all DEA forms that have an error on them must be voided and retained by the pharmacy - MISTAKES CANNOT BE CORRECTED

Drug Addiction Treatment Act of 2000 (DATA 2000)

- DOES NOT allow the prescribing of a schedule 2 controlled substance for the treatment of opioid addiction outside of a registered narcotics treatment facility
