it is reversed and the reverse distributor keeps copy 3 and sends copy 1 and 2 to the pharmacy and the pharmacy sends copy 2 to the DEA acting as the supplier
How are controlled substances breaking, damaged or spilled reported?
it is not considered a loss and if recoverable must be disposed of and a form 41 should be submitted but if it is not recoverable then a form is not needed just inventory update
How often should controlled substance inventory be taken?
every 2 years
What are the storage requiremets for inventory records of controlled drugs?
C2 inventory records kept separate and C3-5 can be stored with or without non controls but require a red C one inch high on the lower right corner
What should the inventory records contain when taking inventory of controlled substances?
date, time, names of substances, dosage form, strength, quantity, number of containers
What are the inventory counting requirements for controlled substances?
schedule 1 and 2 require exact amount, schedule 3-5 containers holding <1000 units can be estimated but >1000 require exact count
How long should all DEA records be maintained?
2 years
What DEA records must be kept on site?
original prescriptions, copies of form 222, inventory records
What DEA records can be kept at a central location and when can you keep them after the DEA receives the request?
shipping and financial records and can be stored 14 days after the DEA receives the request
