Upon examination on Mr K, there is mass in left hemiscrotum. It is possible to palpate the testis, and the mass confined to it. The mass is not fluctuant and not transilluminate. The epididymis and spermatic cord can be palpated as distinct from the mass. The testis not tender to palpation.
Risk factor for testicular tumour
CRYptorchirdism (undescended testis), HIV, gonadal dysgenesis
Clinical presentation for testicular tumour
- painless testicular mass / dull ache in one testis in young man (15-35y/o)
-± history of trauma accompanying discovery of mass
- back pain ( if para aortic nodes infiltrate w/ mets)
Classification of testicular tumour
common : teratoma vs seminoma
others: embryonal carcinoma, leydig cell tumour, sertoli cell tumour (a/w gynaecomastia)
Investigations of testicular tumour
1. US of scrotum : if seminoma (hypoechoic lesion)
2. Tumour markers : LDH, AFP, BhCG
3. Staging : CT AP - assess para-aortic LN involvement
CXR - distant disease
Staging of testicular tumour
Stage 1 : testis lesion, no nodes involved
Stage 2 : nodes below diaphragm
Stage 3 : nodes above diaphragm
Stage 4 : pulm & hepatic mets
Radical orchidectomy via inguinal approach with combination CT
Asymptomatic fluid collection around testicles (processus vaginalis) that translluminate
What is Testicular torsion?
- sudden rotation of testis causing twisting of spermatic cord --> ischemia
-ppt by trauma, cycling
- Investigation: Doppler US (no blood supply to affected testis)

Intravaginal torsion: cord twist inside deeper than tunica V. --> do US doppler still normal BF (flow to T.V.)(preserved flow)
