Directions:
- knock, greet resident
- explain procedure
- wash hands, get equipment, put on table
- check resident's ID
- get warm water (paper towel to turn sink on/off)
- resident checks water temperature
- put washcloths in water
- pull curtain for privacy
- elevate the bed (put rail up on opposite side)
- bath blanket over cover, pull cover down
- untie back of gown
- lay towel across chest
- put gloves on
- wet washcloth for face (inner to outer corner of the eye)
- put washcloth in hamper
- pat face dry with towel and place towel under undressed arm
- wet washcloth with soap for arm
- wash neck, shoulder, chest down to waist, top of arm to fingers, wash back of the arm and end with armpit
- cover patient back up and put washcloth in hamper
- wet washcloth without soap and repeat
- use extra towel to pat dry in the same order (keep the towel)
- lay gown across bath blanket, put on both sleeves, pull bath blanket and old gown down
- old gown into laundry
- cover resident back up with bath blanket
- turn resident to the side (cross arms and legs)
- move towel closer to the resident's back
- washcloth with soap to wash the WHOLE back
- washcloth without soap to rinse back
- towel to pat back dry
- take gloves off, and put lotion on hands
- base of back (heart shape) X3
- mini circles X3
- bace of back (heart shape) X3
- wipe excess lotion with towel
- take strings of gown and tie while resident is on side
- take towels and place in hamper
- roll resident back
- uncross patient and pull sheet back up while removing bath blanket
- throw in dirty laundry
- lower bed back down
- call light within reach
- push curtain back
- bedside back next to the bed and put hamper away
- side rails down
- dump basin water and dry it out
- store soap and lotion in basin
- dry bed table off with paper towel
- wash hands

- toothbrush
- toothpaste
- oral swabs or toothetes
- emesis basin
- cup
- gloves (2 pairs)

Directions:
- knock, greet resident
- explain procedure
- wash hands, get equipment
- put 1 towel in sink
- washcloth as barrier for toothbrush and toothpaste
- cup for toothettes
- take 1 towel and 1 pair of gloves to bedside
- verify resident's identity
- pull curtain for privacy
- put gloves on
- pick up dentures and take to bathroom
- take dentures out of cup and into emesis basin
- paper towel to turn sink on and rinse denture cup out
- put water in toothete cup
- wet toothbrush and put on toothpaste
- brush dentures all over
- place clean dentures into denture cup
- rinse toothbrush out and set down
- clean emesis basin and dry
- paper towel to turn sink off
- take cup of water, emesis basin, toothetes, and dentures back to resident
- place towel across chest
- one toothetes for upper, lower, and one for tongue and throw away in emesis basin
- water to swish and spit
- ask if resident wants to do again
- towel to pat dry, "do you feel dry"
- take gloves off, throw away
- push curtain back
- give call light
- "anything else I can do for you"
- take towel and basin
- throw towel in dirty laundry
- grab another pair of gloves
- throw stuff in emesis basin away and rinse emesis basin and dry
- clean toothbrush again and store for patient
- take towel at bottom of sink and squeeze out, take towel and barrier and place in laundry
- take gloves off and wash hands
- state surveyer I'm done
