What is the Nursing Process?
A systematic and collaborative process that enables the nurse, along with the patient, to assess, determine nursing diagnosis, identify and plan outcomes/care, implement care, and evaluate results.
Where did the nursing process come from?
It has been mentioned since 1955 in various forms, but it was legitimized in 1973 by the ANA (American Nurses Association) congressing for Nursing Practice.
What's the point? why do we use this?
First and foremost, to promote the wellbeing of patients.
Accrediting bodies require this to show our care process for our patients.

Step 1 : Assessment
systemically collect patient data

Step 3 : Plan
Develop a holistic plan of individualized care that specifies the desired patient goals and related outcomes and the nursing interventions that will help the patient meet those expected outcomes.

Follow out the care plan

Evaluate the effectiveness of the care plan in terms of patient goal achievement.
Documentation
-In the world of healthcare, if you didn't document it, it didn't happen. So, we make sure we get it.
-Care plans are updated every shift and as needed. Goals can change on a regular basis and care plans should reflect these changes in patient status.
-They are a way that the interdisciplinary team can communicate and help reach goals.
