Oxytotic agent (X)
1. Increase contractions (0.5-1 unit/min titrated 1-2 units every 15 min)
2. Prevent/treat hemorrhage (10 units IM or 20units IV post-partum)
May cause hyper stimulation. Regulated closely on a pump.
Adverse: tetanic contractions, urinary retention, fetal distress.
Contraindicated in non-vertex presentation, possible uterine scarring.
Methergine
Ergot Derivative (C)
Treats hemorrhage
0.2mg IV/PO
May cause nausea, vomiting, consider waiting 12 hours to breastfeed.
Contraindicated in hypertension and heart disease
Cytotec
Prostaglandin/ GI agent (X)
1. Induces labor
2. Treat Hemorrhage
0.25mcg vaginally every 3-6hours or 800mcg PO
High risk for uterine rupture (prior c/s). May cause tachysystole, diarrhea and chills.
Contraindicated in VBAC
Hemabate
Prostaglandin (C)
Treats hemorrhage
1amp IM
Expect adverse GI, many adverse effects.
Contraindicated in asthma or other chronic condition
Magnesium Sulfate
Antidysrhythmias (D)
1. Prevent/ stop seizures
2. Tocolytic
4-5gram loading dose followed by 1-3 grams an hour
4-6 gram loading dose followed by 2-4 grams an hour
May cause flu like symptoms. Do not give more than 5-7 times a day
Contraindicated in renal impairment, hypercalcemia and mag toxicity
Calcium Gluconate
Antidote for Mag Sulfate (C)
1.5--3gram IV
May cause hypotension or diarrhea. Not given IM/SC
Narcan
Opioid reversal (C)
Adults: 0.4-2mg
Babies: 0.01 mg/kg
IV/IM/SC/ET
May cause nausea/vomiting and increased BP. Contraindicated in cardio disease, seizure disorder.
Fentanyl
Synthetic opioid (C)
Given for pain
25-100mcg IV Q 1 hr x 3 doses
May cause confusion, dizziness, nausea, dry mouth. Contraindicated with MAOI use.
Demerol
Synthetic opioid (B)
Given for pain
5-100mg IM Q1-3 hours
May cause agitation, dizziness, and dry mouth.
Contraindicated in renal or hepatic impairment.
Nubain
Opioid analgesia (B)
Given for pain
10-20mg IV, IM, or SC Q3-6 hours
May cause sedation, N/V, dizziness and headache.
Contraindicated in renal or hepatic impairment.
Stadol
Opioid analgesia (C)
Given for pain
1-2mg IV/IM Q4hours
May cause sedaion, N/V, dizziness, headache.
Contraindicated in respiratory depression.
