Definition
- highly emetogenic (pre-medicate and use aggressive antiemetic therapies)
- highly reactive (no aluminum needles or hubs)
- inactivated by alkaline solutions (e.g., sodium bicarbonate)
- dose paclitaxel at least 24 hours before cisplatin (cisplatin delays excretion of paclitaxel)
Nephrotoxicity:
- highly nephrotoxic
- fluid status is critical; give 1L 0.9% NaCl pre- and post-dose and maintain fluid status
- use caution or avoid use in patients with renal dysfunction
- nephrotoxicity may be delayed and occur up to 2 weeks after infusion
