cisplatin

cisplatin
- highly emetogenic (pre-medicate and use aggressive antiemetic therapies)
- highly reactive (no aluminum needles or hubs)
- inactivated by alkaline solutions (e.g., sodium bicarbonate)
- dose paclitaxel at least 24 hours before cisplatin (cisplatin delays excretion of paclitaxel)

Nephrotoxicity:
- highly nephrotoxic
- fluid status is critical; give 1L 0.9% NaCl pre- and post-dose and maintain fluid status
- use caution or avoid use in patients with renal dysfunction
- nephrotoxicity may be delayed and occur up to 2 weeks after infusion
carboplatin
- use caution in patients with abnormal renal function (reduce dose)
- highly reactive (no aluminum needles or hubs)
- contraindicated in patients allergic to cisplatin, platinum, and mannitol
methotrexate
- carbonated beverages can increase the acidity of urine, resulting in impaired drug elimination
capecitabine
- take with a full glass of water 30 minutes after a meal
- contraindicated in patients with known DPD deficiency
- renal dosing required
gemcitabine
- if given with cisplatin, be extra vigilant in monitoring for myelosuppression and electrolytes (especially K, Mg, and Ca)
- reduce dose for renal or hepatic imapairment
