1). Gather meds from the cart and preform initial #1 check. Confirm which eyes are getting the med.
2). Perform second check outside the door

5). Have tissues ready to remove solution and tears
6). Clean eyelids and eyelashes with cotton square that has water or NS on it. . Move from the inner corner to the outer corner and only use each part of the square once
7). Tilt the patient's head back if they're sitting or place a pillow under their head if they're lying down. You can also tilt their head towards the effected area to prevent the solution from getting into the other eye

8). Remove the cap from the medicine bottle and don't touch the inner side of the cap or the tip of the bottle
9). Invert the monodrip plastic container which is commonly used to instill eyedrops
10). Have the patient look up while focusing on something on the ceiling
11). Place the thumb OR two fingers near the margin of the lower eyelid immediately below the lashes and apply pressure downward over the bony prominence of the cheek. The lower conjunctival sac is exposed as the lower lid is pulled down

12). Hold the dropper close to the eye but avoid touching the eyelids or lashes. Use the lateral side of your hand to rest on the patient's forehead, right above the eyebrow as needed for stability
13). Squeeze the container and allowed the prescribed number of drops to fall to the lower conjunctival sac. Do NOT get med in the cornea
14). Release the lower lid after the eyedrops are instilled and ask the patient to gently close their eyes
15). Apply gentle pressure with your gloved finger over the inner canthus to prevent the eyedrops from flowing into the tear duct

20). Assist the patient to a comfortable position, put the bed in the lowest position, make sure the call bell is within reach
21). Clean the area as needed
22). Wash hands
23). Document

19). Evaluate patient's response to the medication
