Drug: May increase bleeding when taken ith NSAIDs, anticoagulants, antineoplastics, azole antifungals, barbituates; intereferes with metabolism of phenytoin, warfarin, fluvastatin, tamoxifen, tolbutamide, NSAIDs, torsemide
Lab: prolongs bleeding time
Herb: may increase bleeding when taken with ginger, garlic, ginkgo, feverfew, green tea
Distribution: PB: 94%-98%
Metabolism: t1/2 8 h
Excretion: 50% in urine and 50% in feces
PO: Onset 1-2 h
Duration: 3 d or longer
Therapeutic Effects/ Uses
To prevent recurrence of MI, stroke; to prevent vascular death.
Mode of action: inhibits platelet aggregation. Prevents ADP from binding with ADP platelet receptor.
Abdominal pain, flulike symptoms, dizziness, epistaxis, HA, fatigue, chest pain, diarrhea.
Life-threatening: Agranulocytosis, aplastic anemia, thrombocytopenia
is an antiplatelet (used to prevent thrombosis in the arteries by suppressing platelet aggregation) drug frequently used after MI or stroke to prevent a second event. It may be prescribed singly or with aspirin. It has been stated that Plavix and aspirin are more effective in inhibiting platelet aggregation together than if used as separate antiplatelet therapies.
Assess: thrombotic/thrombocytic purpura, fever, thrombocytopenia, neurolytic anemia; Symptoms of stroke, MI during treatment; Hepatic studies: AST, ALT, bilirubin, creatining (long-term therapy); Blood studies: CBC, differential, Hct, Hgb, PT, cholesterol (long-term therapy);
Evaluate: therapeutic response: absence of stroke, MI;
Teach Pt/Fam: that blood work will be necessary during treatment; to report unusual bruising, bleeding to prescriber--teach that it may take longer to stop bleeding; to take w/o regard to food; to report diarrhea, skin rashes, subQ bleeding, chills, fever, sore throat; to tell all HCPs that clopidogrel is being used; may be held for 5 days before surgery
