Patient complains of a history of widespread pain in the low back with/without radiation to buttocks and legs and pain in neck and across shoulders. The pain involves both sides of the body, above and below the waist as well as the axial skeletal system and has been present for at least three months.Patient complains of fatigue, sleep disturbance, morning stiffness, paraesthesiae, feeling of swollen joints but with no objective swelling. Patient also complaints of memory disturbance, difficulty word finding, headaches, light-headedness and dizziness. They report fluctuations in weight, anxiety and depression.
