List the rules of Asepsis
- all personnel must know what sterile is and what is non-sterile
- sterile personnel touch sterile things, and non-sterile personnel touch non-sterile things
Identify general guidelines of Asepsis
- observe routine hygienic measures
- keep nails cut short and filed smooth, do not wear polish
- wear clean shoes
- remove all jewellery
- a cap, mask, and scrubs or gown must be worn in surgical room when packs have been open- lab coats removed
- traffic in and out of surgery suite should be minimized
- surgery door should be kept closed
- very important to wash hands between every surgery and between every animal that you handle
Describe methods used for surgical scrubbing
Timed Method- where the entire hand and forearm of each hand is scrubbed in a systematic fashion, for a prescribed amount of time, minimum is 5 mins
Systematic Method- where each area of the hands and forearms are scrubbed a specific number of times eg. 10 x per finger
Describe how to properly put on a gown
- dry hands
- pick up gown
- let gown unfold
- open to locate sleeves
- slip arms into sleeves
- hold arms out and slightly up
- tech helps put gown on
gown is only considered sterile from the waist up to the neck and on the front only and from 2 inches above elbow to the cuff
Explain the sterile field in regard to your person
Only sterile from the waist up, arms must be above waist level
Describe the rules of sterile field etiquette
- "Back to Back": when passing another gowned person you must pass them back to back
- sterile area = only sterile area of gown may be in contact
- if you turn your back on a sterile tray, it is considered contaminated
- if a non-sterile person is too close, you are not sterile anymore
Explain the procedure for open gloving and for closed gloving
Open gloving- hands are extended through sleeves of the gown ad the gloves are pulled up over the cuffs
Closed gloving- the hands are held within the sleeves and the gloves are pulled over the cuffs with no exposure of skin to environment
Describe how to open packs and how to move around sterile field
Open Packs: edges = non-sterile, unwrapped by opening the flap farthest away first, the sides next, and the nearest flap last. Open on a clean, flat, dry surface in surgery room
Moving Around: be aware of sterile and non-sterile surfaces, non scrubbed personnel should always face surgical field, non scrubbed personnel should not step between two sterile fields or scrubbed personnel
Explain how to pass instruments to surgeon
Scalpel: hold scalpel handle with blade facing away from hand and point towards assistant
Ringed: place handle of instrument firmly into hand of surgeon, or pass body of instrument to surgeon- sharp area pointed up. Ratchets closed
ALWAYS use needle holders or hemostats to place blades
Explain how to handle live tissue
Assistants fingers may be used as clamps to minimize trauma to tissue, exposed tissues should be kept moist, can use a retractor but not too much pressure should be used.
Describe how to maintain homostasis during the procedure
- Blotting areas of hemorrhage with gauze helps visualize the surgery site
- Bleeding tissues should be blotted, not wiped, wiping can cause clotting that has started to be wiped away. It also causes friction and irritation which can lead to inflammation.
Identify the amount of blood that each gauze size can hold and explain what the gauze count is
A 3x3 gauze holds about 6mL of blood
A 4x4 gauze can hold about 10mL of blood
- gauze should be counted before and after surgery to make sure none have been discarded in the patient, if one is missing you must find it.
