The health continuum is a scale that we may use to assess our overall health. Early mortality and an excellent Level of Health are the criteria. A continuum of care contains a wide variety of health services in order for treatment to change with the patient through time. The continuum of care was created with the concept that a service user is most susceptible during gaps in treatment, and it serves to guarantee that those gaps are addressed.
