VerifiedTo carry out this order the first thing to consider is the immobilization of T.W. since it is an emergency, it is suspected that there is a spinal cord injury that happened, and to prevent further damage to the spinal cords, immobilization is needed. Next, it is important to maintain patient’s airway system, hence oxygen must be given as soon as possible because hypoxemia may be formed which can worsen a neurologic injury in the spinal cord. Next to this is ECG monitoring since hypoxia and hypotension must be prevented in patients with spinal injury. When the patient is already stable, the initiation of two large bore IVs will be done for the administration of medication. This is then followed by neurologic assessment every hour to monitor T.W. for any symptoms related to progressive neurologic damage. The application of warming blankets are needed since peripheral temperature sensations cannot reach the hypothalamus for patients impaired with spinal cord injury.
The following actions are to be carried out following the physician's orders:
a.) Immobilize the cervical spine to minimize pathological damage to the spinal cord by the vertebrae.
b.) Administer oxygen at 4 L per nasal cannula, a spinal cord injury can lead to an impairment of the respiratory system and mortality when left untreated.
c.) Conduct ECG monitoring, a traumatic spinal cord injury can lead to cardiovascular complications.
d.) Initiate two large bore IVs for possible administration of medications and fluids for volume replacement.
e.) Conduct a neurologic assessment every hour to determine the level of injury and extent of functional impairment. This is considered vital for treatment selection for the patient.
f.) Insert Foley catheter to drain bladder contents. Urine output must be measured hourly.
g.) Apply warming blankets as needed. Patients with spinal cord injuries have decreased capability to regulate body temperature.
