When we talk about nuclear transmutation reactions, these are the reactions where some nucleus reacts with some other nucleus or subatomic particle to form a new nucleus which has higher mass and higher energy (it is more unstable) than the starting nucleus.

When we talk about nuclear decay reactions, these are the reactions where some nucleus is emitting radiation to form a new nucleus (or more than one) which has lower mass and lower energy (it is more stable) than the starting nucleus.
