- Ascending: retrograde ascent of bacteria up urethra (bacteria from large bowel can colonise perineum and ascend to bladder) (may also be further ascent to the kidneys from the bladder via the ureter) - Women have much shorter urethras than men which means the bacteria from the perineum have a shorter distance to reach the bladder so an infection is more likely.
