A mother comes in with her 3yo son, he is unwell and needs something to help him sleep better at night. She has seen the doctor, and he said just let him rest.
What do you suggest?
voltarin rapid 12.5mg tablets
An elderly woman (70+) comes in with back pains after gardening and wants something to help. has tried ibuprofen with no effect.
she is not on any other medications and has no known medical conditions.
DISPRIN (Asprin) is no longer recommended for general use or to be given to children. Paracetamol (PANADOL) is much safer and has less side effects long term. for the children, a Paracetamol suspension (PAMOL) would be suitable as it is nicer on their stomachs.
A Woman in her 30s has come in asking for Disprin for general use for her and her young family (5&8)
what would you suggest.
Tension headache - trying a combination of paracetamol and ibuprofen (Nuromol)
A woman in her 50s comes in with a headache. she describes it as tight band around her forehead that started yesterday and hasn't got better. Hasn't tried anything. She has never had anything like this before. she has no medical conditions and isn't taking any medication.
migraine - either ibuprofen (Nurofen) or a combination of ibuprofen and paracetamol (Nuromol) and ride it out. rest in a quiet, dark space and keep up fluids.
EXTRA - REFER
A man in his late 20's comes in seeming very unwell. he says he has an awful headache on one side of his head that started this morning. tired Panadol. He has experienced nausea, is sensitive to light, and seems to have stars in his eyes (visual disturbances). he has experienced something similar before but it didn't last long and wasn't this bad. no medications or medical conditions.
EXTRA - 60's, came on suddenly = one hour, nothing has worked, never had anything like this before
sinus headache - paracetamol to help with pain (okay for anti depressants), try a decongestant nasal spray (otrivin) (3 days on 3 days off, dispose of after 90 days) steam can help, and something soothing like vicks.
if no improvement within the next 3 days, come back or see doc.
Woman in her early 20's comes in with a sore head. she describes tight feelings under her eyes and around her nose. She has had it for the last 2 days. she hasn't tried anything yet. she has no allergies. Only medication is an anti depressant so she can not take ibuprofen. no medical conditions.
REFER to pharmacist or doctor. danger of head injury
= sudden onset
= dizzy and nauseous
= memory loss
a 18yo man comes in asking for some sort of pain killer, he has a headache and has visual disturbance's, dizziness and nausea. It started a hour or so after his rugby game this morning (had a knock to the head during). says he hasn't tried anything BUT when shown ibuprofen, he says he has taken that. no medications or medical conditions.
Naprogesic (naproxen 275mg) fast acting, designed for period pain, works after pain has begun.
2 tabs w food followed by 1 each 6-8 hrs as required (max 5 in 24hrs)
a young girl comes in with abdominal cramps. After further question its discovered she is on her period, its been going on for a day or so, has tried Nuromol and it didn't work, wants something stronger. no allergies, no medications, no medical conditions.
Ponstan - only works BEFORE pain has begun, designed for period pain, patient hasn't tried it yet.
A lady in her late 20s comes in asking for something for period pains. she is not on her period yet but should be next week. in the past she has tried nurofen, panadol, and nuromol, also naprogestic (made her feel sick). she wants something else. no allergies, no medications, no medical conditions.
Day/night cold and flu tablets, (night tablets have sedating antihistamine to help sleep), otrivin adult nasal spray (decongestant = 3 days on, 3 days off), difflam sore throat lozenge for the sore throat.
EXTRA - refer and no cold and flu tablets (contain phenylephrine) nasal spray, lozenge nasal spray technique
A 30yo man has a cold and needs something to help him sleep as he is a night taxi drive. He mentions having a blocked nose, and a sore throat which hes had for the past 2 days. has tried Otrivin clear, his wife suggested it, it didn't help. no allergies, no medications and no medical conditions.
EXTRA - All the same BUT taking blood pressure medication
otrivin adult nasal spray (decongestant = 3 days on 3 days off) try steaming or vicks to help clear and relax the nasal passages/sinus.
EXTRA - otrivin clear (saline nasal spray) and steaming nasal spray technique go to a specialist
25yo woman keeps getting sinus headaches and wants something to help. she has congested sinus' and a blocked nose. she hasn't tried anything, no allergies, no medications, no medical conditions
EXTRA - all the same but she may be pregnant
durotuss dry cough liquid for during the night (more accurate measuring + conical measuring flask) and durotuss dry cough lozenges for during the day. make sure you don't double dose, wait for the past dose to run out before the next. The expectorant wont help as its for a different cough.
EXTRA - REFER
A 20yo man has a cough. he describes it as irritating and it annoys everyone at his work, he's had it for 3 days he tried the old cough mixture he had light green lid (durotuss expectorant liquid) it didn't work. no allergies, no medications, no medical conditions
EXTRA - lasted about 7 days
durotuss chesty cough tablets or chesty cough liquid.
EXTRA - refer, could be a chest infection
- chesty cough liquid is often better for an upset stomach
a 25yo lady comes in with a deep nasty cough. She's had a cold but can't shake the cough. The cold lasted 4 days but the cough has been around for the last 2 days. hasn't tried anything for the cough, hasn't taken anything within the last day. no allergies, no medications, no medications.
EXTRA - the flehm is coloured dark green.
- she's tried the tablets and it upset her stomach.
Try a throat spray (difflam is anti inflammatory not anti septic) and possibly a betadine sore throat gargle is anti bacterial so should help avoid extra bacteria growth = shouldn't get worse.
a 40 yo man comes in with a sore throat that he's had for the last 2 days, he describes it as scratchy but not painful. Has tried antiseptic lozenges and they made him feel weird. no allergies, no medications, no medications
REFER - seems to be strep throat. go see the doctor and get a throat swab, don't share food or drink and don't go to school. Rest.
A mother comes in with her 12 yo daughter, she has a sore throat, and no other symptoms she thought it was a cold but she has had the sore throat for 4 days with no improvement and isn't sure. the girl describes it as painful, saying it hurts to swollow. she has tried some lozenges and paracetamol but it didn't help. no allergies, no medications, no medications
difflam anaesthetic lozenges
will help numb the throat, take care with hot foods and drinks.
a 50 yo man comes in asking for something to help his very sore throat. he has been to the doctor as it is not strep throat. the doctor suggested a lozenge. he has tried one saying anti inflammatory on the box, and found it didn't help. no allergies, no medications, no medications
kiwi herb destuff for the sniffle, and chesty cough liquid for the cough.
keep and eye on it, if I gets worse go to see a doctor keep using pamol to help him sleep and will also help avoid fevers

unfortunately sounds like the flu. go to the doctor and get a test done. isolate and don't go to work or school. avoid extra contact. rest and hydrate.

heartburn due to the pressure of the baby, gaviscon liquid or tablets. take 2 hours after food. don't lie down straight after eating and avoid spicy food.
a 20yo man comes in with an upset stomach, he describes it in his lower stomach, he had some spicy foods the night before then felt unwell this morning. no vomiting or diarrhea. tried just drinking more water and it didn't work. no allergies no medications and no medical conditions.
lower stomach indigestion, mylanta tablets (chew or suck) or liquid will neutralize the stomach acid. 1 hour after food. avoid spicy foods or try only mild foods.
a new mother comes in with her 1 month old, he seems to be unsettled, spilling when feeding and will projectile vomit once or twice each day. been going on for the past 2 weeks and the midwife told her it was normal but she is worried. hasn't tried anything, no allergies, no medications, no medical conditions.

dulcolax tablets, take 2 at night (works in 8hrs) the 1 as needed. increase fibre and water intake. if no improvement in 24 hrs go to the doctor = faecal impaction. try and go to the bathroom when you feel you need to, don't wait
